Distal coronary anastomosis

After CPB institution, the heart is luxated to inspect the coronary arteries. When a surgical target is identified, the arteries are exposed. After positioning an aortic root vent, the aortic cross clamp is applied and the cardioplegia is delivered. The coronary arteriotomy is made to match the venous diameter, and the anastomosis is performed with a running 7/0 suture with a parachute technique. An oxygen blower is utilized to obtain a bloodless field. When the anastomosis is done, cold cardioplegia is delivered through the graft, to detect also any leak from the suture. A latero-lateral anastomosis is utilized to create a sequential coronary artery bypass.
